We are a collaboration of 7 GP practices across Chorley and Leyland and serve a population of apx 38000 patients. Together we form Chorley Together Primary Care Network (PCN)
As a PCN we support the operational, strategic and financial resilience of Core Practices in order to improve the health and wellbeing of our population. We work in collaboration with health, social care and third sector providers, to support people holistically from birth to the end of their lives. We strive to drive improvements in local healthcare, build strong relationships and promote integrated working for the benefit of our population.
