Extended Access service
Monday – Friday 18:30-20:00 and Saturday 09:00-17:00. Appointments are available to all patients across our PCN and are delivered from our practices as follows
Monday – Whittle Surgery and Dr Jacobs
Tuesday – Dr Dawoud’s
Wednesday – Withnell HC
Thursday – The Surgery, Chorley
Friday and Saturday – on rotation across our practices.
All sessions provide a mix of face to face and telephone appointments with a healthcare professional, E.g GP, Practice Nurse, HCA.
Social Prescribing
We work in conjunction with Chorley Council and offer a social prescribing service to our patients. Patients can refer directly to the service or referral can be made by a member of your practice team.
Pharmacists and Pharmacy Technicians
We have a pharmacy team supporting practices across our PCN with medication reviews and minor ailments.
Care-Co-ordinators
Care-Co-ordinators work within practices across the PCN to provide support to practices within the PCN to manage, identify and provide coordination of care for the PCN population.
Mental Health Practitioner
Our Mental Health Practitioner provides a combined mental health consultation, advice, traige and liaison function to patients across the PCN, supported by Lancashire and South Cumbria NHS Foundation Trust.
They work with patients to:
- – support shared decision making about self management of their condition
- – facilitate onward access to treatment services
- – provide brief psychological interventions, where appropriate
GP Assistants
Our General Practice Assistants provide a support role, carrying out administrative tasks, combined in some areas with basic clinical duties, helping to free up GPs time and contribute to the smooth running of appointments, improving patients experience in our practices.
